Dental coverage is available to you and any of your dependents. You may choose the Dental Option that best meets your personal and family needs. Up to three options may be available to you, depending on where you live or work:

All associates can choose from these two options:

1.   Dental Basic — A traditional dental plan with optional Dental Preferred Provider Organization (Dental PPO) feature.

Dental Plus — Enhanced traditional dental plan with optional Dental Preferred Provider Organization Dental PPO feature.

If your home or work zip code is in the network, you may be able to choose from a third option:

3.   DMO (Dental Maintenance Organization) — A managed dental care plan which requires the use of a primary care dentist within the network.

The Dental Basic option provides benefits for preventive and basic services. The Dental Plus option provides benefits for preventive, basic, major, and orthodontia (braces) services for adults and children. The DMO option provides coverage for preventive, basic, major, and orthodontia services for dependent children when treatment begins before age 19. The three options differ in the percentage of eligible expenses and the services each covers. The Dental Basic and the Dental Plus options differ from the DMO option in how services are provided.
